Please be advised, it is advisable that before you start to fill out the online application form below, you should also be aware of the following:
- Make sure that before filling the form that the page has completely loaded, as this page may take a while to load depending on your current internet connection. To check just make sure you see the submit registration button at the end of the page. You may need to scroll down to see this.
- In addition please be advised that if you make any errors when filling the form, you will be advised of the errors when you click the submit registration button. You will need to scroll to the top to view the errors or depending on your browser the fields will be highlighted in red. Make sure that before you re-submit, you have checked all the fields as some of the selection fields may revert to their default value.
- Before submitting the form you should agree to the Terms/Conditions just above the submit Registration button as well as making sure you entered the ReCAPTCHA text/numbers from the generated image above where it says “Type the text”.
- Please also be aware that if you have problems completing the online form, you have an option of download an offline PDF version by clicking here, which can then be fill out via Acrobat Reader and printed out to be handed to us personally or emailed directly.
